We provide transition-focused services that are student-centered, strengths-based, and aligned with federal and state expectations.
StrataBloom Consulting works collaboratively with students with disabilities ages 14–22, their families and caregivers, public and charter schools, transition teams, special education staff, and community agencies. Our approach centers the student while ensuring families and partners remain informed, supported, and aligned throughout every stage of the transition process.
